claude code agent 227 d0eae69086 fix(ai): raise reindex pre-seed TTL to the client poll cap; cover predicate clause; align docs (F11-F13)
F11: PRE_SEED_TTL_SECONDS 45->120 (= client REINDEX_POLL_CAP_MS). At concurrency
     1 a queued reindex can wait past the old 45s; if the pre-seed expired while
     pending, getMasked fell back to the COUNT and reported done, so the client
     stopped polling and missed the climb. Tie the pre-seed TTL to the client cap.
F12: extend the lockstep integration spec — insertPage takes content; a
     text_content=null + text-node-content page is IN and a math-only page is OUT,
     pinning the structural "type":"text" clause (and the jsonb space-after-colon).
F13: list all three embeddable clauses in the reindex JSDoc/inline comments.

claude code agent 227 85b38d6946 fix(ai): address reindex-progress review round 1 (PR #242)
F1: clear the "Reindex now" spinner once the poll cap fires. Gate the
reindexing part of the button's loading state on the active poll window
(reindexDeadline !== null) so a run that outlives the 120s cap no longer
leaves the button stuck-disabled with a stale `reindexing: true`; the
admin can restart.

F2: rewrite reindexWorkspace JSDoc to describe the EMBEDDABLE page set
(text OR existing embeddings), matching getEmbeddablePageIds /
countEmbeddablePages instead of the old "every non-deleted page".

F3: extract the shared embeddable-content predicate into a private
PageRepo.embeddablePredicate helper, called by both countEmbeddablePages
and getEmbeddablePageIds, removing the verbatim duplication. Behavior is
identical (lockstep int-spec stays green).

a 95d07d8d6f fix(ai): align reindex live denominator with the steady-state count
Review fixes for the reindex-progress counter (#242):

1. Denominator jump (478 -> 500 -> 478): reindexWorkspace iterated
   getIdsByWorkspace() (ALL non-deleted pages) but the seed/status use
   countEmbeddablePages (text OR existing-embedding), so the live total exceeded
   the steady-state total whenever empty/text-less pages existed. Add
   PageRepo.getEmbeddablePageIds() that selects the IDs of the EXACT same set
   countEmbeddablePages counts (deletedAt IS NULL AND (text_content matches a
   non-whitespace char OR an EXISTS non-deleted pageEmbeddings row)), and have
   reindexWorkspace iterate THAT set with total = its length. Iteration set and
   count source change together, so done reaches exactly total == the
   steady-state denominator. Dropping text-less pages is correct (reindexPage
   no-ops on them; a page that lost its text but still has stale embeddings is in
   the set via the EXISTS clause and still gets its stale rows cleared). Removed
   the contradictory "worker overwrites with the real page count" / "denominator
   matches" comment.

2. Mid-run re-trigger reset: reindex() unconditionally re-seeded done=0 before an
   enqueue that de-dupes a running job, so a second click/admin/tab reset the
   visible counter while the worker kept incrementing. Now seed only when
   get(workspaceId) === null; the worker's own start() remains the single
   authoritative reset.

3. TTL: documented that it is intentionally tied to write progress
   (start/increment) and never refreshed on get(), so a dead worker's record
   can't be kept alive forever by client polling.

Tests: new embedding-reindex-progress.service.spec.ts (fake ioredis: hash ->
ReindexProgress, malformed/missing/non-numeric -> null, non-finite startedAt ->
0, hgetall throws -> null, start/increment issue hset/hincrby+expire and swallow
Redis errors); reindex() seed order + no-reseed-when-active guard; getMasked
live test now uses progress.total=500 vs DB 478 to pin the progress branch;
indexer specs updated to mock getEmbeddablePageIds.

a 72bb03918d fix(ai): show live reindex progress in semantic-search settings
The "Indexed X of Y pages" counter stayed stuck at "478 of 478" during a
manual "Reindex now" run instead of resetting to 0 and climbing. The status
reports indexedPages = countIndexedPages (DISTINCT pages with >=1 embedding
row), but reindex hard-replaces each page in its OWN small transaction, so
nearly all pages always have rows -> the count never drops.

Add a per-workspace live reindex-progress record in Redis (reusing the
existing global ioredis client via RedisService, no new Redis config):
- EmbeddingReindexProgressService: start/increment/clear/get over a Redis hash
  with a 1h TTL self-clean; all best-effort/cosmetic so a Redis failure degrades
  to the existing DB-count behavior.
- AiSettingsService.reindex seeds {total, done:0, startedAt} at enqueue time so
  the very first poll already reports done=0.
- EmbeddingIndexerService.reindexWorkspace overwrites total with the real page
  count at start, increments done per processed page (success or handled
  failure), and clears the record in a finally (covers success, fatal abort,
  and the unconfigured early-return) so a failed run never sticks.
- AiSettingsService.getMasked returns the live run numbers when a progress
  record is active (plus an optional reindexing flag), else falls back to
  countIndexedPages/countEmbeddablePages.

Per-page edits (reindexPage) never touch the workspace progress record, and no
mass up-front delete is introduced (search availability preserved).

Tests: indexer sets/increments/clears progress (incl. fatal abort and
unconfigured early-return); status reports run progress when active and falls
back when not.

claude_code f543e79c3e fix(ai-embedding): abort bulk reindex on fatal provider errors
reindexWorkspace isolated every per-page failure, so an invalid/missing
API key (401 "User not found") made all pages fail identically while the
batch kept issuing hundreds of doomed requests against the provider.

Add isFatalProviderError() (401/403 auth, 402 billing) and abort the
whole batch on such errors; 429 rate-limit and embedding timeouts stay
per-page isolated. Adds unit tests for the predicate and a regression
test for the abort/iterate control flow.

vvzvlad c8e41e8916 feat(ai): hybrid RRF retrieval, heading-breadcrumb chunks, merged search tool
Improve agent RAG quality with three changes, plus a roadmap doc for the rest.

- Indexer: prefix each chunk with its heading path ("Page > H1 > H2"), built by
  walking the ProseMirror JSON (heading nodes) so a `#` inside a fenced code block
  is never mistaken for a heading. Falls back to plain-text chunking on any error.
  buildChunkRows: drop indexOf-against-source offsets (breadcrumb prefixes break
  verbatim matching) for a cumulative cursor — offsets are provenance-only.
- Hybrid search: new migration adds a generated `fts` tsvector column + GIN index
  to page_embeddings (same english+f_unaccent config as pages.tsv). New
  PageEmbeddingRepo.hybridSearch fuses cosine + full-text rankings via Reciprocal
  Rank Fusion (k=60, equal weights) in one SQL query at chunk granularity.
- Tools: collapse semanticSearch + searchPages into one hybrid `searchPages` tool
  with a query-rewrite-oriented description; gracefully falls back to the REST
  full-text path when embeddings are unconfigured. Access control (space scope +
  page-permission post-filter) preserved. Add a query-rewrite hint to the default
  system prompt.
- docs/rag-improvements-plan.md: record what shipped and the deferred backlog
  (reranker, attachment indexing, eval harness, tuning).

Note: requires a corpus reindex to populate breadcrumbs on existing pages.

vvzvlad 80c900eb54 fix(ai): make RAG indexer observable and bound hung embedding calls
The bulk embedding reindex could hang on a single page forever
("Indexed 27 of 34 pages") with zero log output:
- all progress logs were debug-level, suppressed in production (pino info);
- embedMany() had no timeout, so a slow/hung embeddings endpoint blocked
  the sequential per-page loop indefinitely.

Changes:
- ai.service.embedTexts: bound embedMany with AbortSignal.timeout
  (configurable via AI_EMBEDDING_TIMEOUT_MS, default 120000ms); on timeout
  throw a clear, greppable message, classified by both signal.aborted and
  the error name (TimeoutError/AbortError/ResponseAborted) so a real
  provider error racing the timer keeps its diagnostics.
- embedding-indexer.reindexWorkspace: promote lifecycle/progress logs to
  info; log "[i/N] indexing page <id>" BEFORE the await so a hang names the
  stuck page; warn on slow pages (>30s); add timing + final summary.
- .env.example: document AI_EMBEDDING_TIMEOUT_MS.

vvzvlad b46aed53e3 feat(ai): surface provider error bodies + probe embeddings in test connection
A misconfigured embeddings endpoint failed the RAG indexer with an opaque
"Invalid JSON response" and was not caught by "Test connection" (which only
probed the chat model), so it only surfaced silently during background
indexing.

- add describeProviderError(): formats AI SDK errors as
  "<statusCode>: <message> | response body: <truncated one-line snippet>"
  (statusCode/message/responseBody never carry the API key)
- use it in the bulk-reindex catch and the embedding processor's formatter so
  the real cause (e.g. an HTML 404 from a wrong base URL) is visible in logs
- testConnection now probes chat AND embeddings independently: skips a probe
  when that capability is unconfigured, returns ok:false with a Chat:/Embeddings:
  prefix on real failure, "not configured" when neither is set

vvzvlad 52e19fe678 feat(ai): wire up workspace RAG bulk reindex + manual "Reindex now"
The WORKSPACE_CREATE_EMBEDDINGS / WORKSPACE_DELETE_EMBEDDINGS jobs were
enqueued (on AI Search enable/disable) but had no AI_QUEUE handler, so
existing pages were never indexed ("Indexed 0 of N pages") and disabling
never purged embeddings.

- EmbeddingProcessor: handle WORKSPACE_CREATE_EMBEDDINGS (bulk reindex all
  live pages) and WORKSPACE_DELETE_EMBEDDINGS (purge workspace embeddings)
- EmbeddingIndexerService: add reindexWorkspace() (skips when embeddings
  unconfigured; per-page error isolation) and removeWorkspace()
- PageRepo.getIdsByWorkspace(), PageEmbeddingRepo.deleteByWorkspace()
- AiSettingsService.reindex() + admin-only POST /workspace/ai-settings/reindex
- Frontend: "Reindex now" button, service call and mutation
- Stable per-workspace jobId with remove-before-add so a stale job can't
  block future reindexes; cancel the delayed purge on enable/reindex so it
  can't wipe freshly-built embeddings

vvzvlad 65f0713a70 fix(ai-chat): live streaming, open-page context, any-dimension embeddings" -m "- streaming: give useChat a STABLE store id (chatId ?? per-mount generated)
so the v6 hook stops re-creating its store every render on a new chat
  (which wiped the optimistic user message + streamed deltas, so nothing
  showed until the turn finished). Also send X-Accel-Buffering:no + flushHeaders.
- context: client sends the currently-open page {id,title}; the system prompt
  tells the agent which page 'this page' refers to (it reads it via its
  CASL-scoped getPage tool; id is prompt-context only, no server-side fetch).
- embeddings: make page_embeddings.embedding dimension-agnostic (drop the
  HNSW index + ALTER to vector), remove the hard 1536 guard, filter search by
  model_dimensions — so 3072-dim (and any) models index instead of being
  skipped. Seq-scan <=> search (wiki scale); existing pages reindex on next edit.

vvzvlad a4b7919753 fix(ai-chat): OpenAI Chat Completions for multi-turn + provider settings, stream UX & errors" -m "Live-stand fixes (OpenRouter / OpenAI-compatible):
- openai provider: use .chat() (Chat Completions) instead of the default callable
  (Responses API), which gateways reject on multi-turn -> 400.
- updateAiProviderSettings: assemble settings.ai.provider via jsonb_build_object
  with ::text-cast bound params + jsonb_typeof self-heal (postgres.js was
  double-encoding it into an array; the ::text cast avoids 'could not determine
  data type of parameter').
- chat agent: drop the hard maxOutputTokens cap (truncated complex tool calls);
  keep a tiny cap only on the test-connection ping.
- testConnection + chat stream: surface the real provider error (statusCode+message)
  to logs and the UI instead of generic masks; never log the API key.
- chat UI: typing indicator, incremental streaming render, tool 'running' status, Stop.

Also bundled (prior uncommitted ai-chat work):
- history 'AI agent' provenance badge; vector RAG (pgvector image + page_embeddings
  + AI_QUEUE indexer + space-scoped semanticSearch); external MCP servers backend
  (@ai-sdk/mcp client, SSRF IP-pinning, encrypted headers, admin CRUD/Test);
  yjs duplicate-instance fix via pnpm patch (single CJS instance server-side).
